How to apply
There is no special form for grant applications. However, each proposal must include a one-page cover letter describing the project, the amount of funds requested, a statement that the applicant is not a 509(a)(3) agency, the contact person and email address. The proposal itself should be no more than four pages, written in microsoft word or excel and include 1) project description, purpose and outcome desired, 2) justification of need, 3) budgeting and expenditure of funds, 4) specific goals, objectives, outcomes and methods of meeting them, and 5) time frame of completion.
